    Hospital/surgical   deductibles/coinsurances
    Guaranteed issue Pre-existing conditions covered
    Spreadsheet enrollment
    Outpatient Surgical covered at 50% base coverage
    X-Rays covered at 50% base coverage
    Outpatient Lab Tests  subject to overall deductible
    Benefits exclude Mental Health deductibles/coinsurances
    Accident  & Critical Care Coverage’s-Optional
